Where do I get the application form for the Chinese Government Scholarship?

The guide names the form as item 1 of section VI, "Application Form for Chinese Government Scholarship (filled in online)". Step 4 of section V says you fill in and submit that form online through the Chinese Government Scholarship Information System, linked from campuschina.org. This page explains the order of steps, what to prepare, and how one programme's online system works on screen.
The order of steps
Section V of the CSC application guide sets the order. Read the steps as one sequence:
- Learn about the designated universities and majors at campuschina.org or by other means, then choose the programme, university and major.
- Reach the designated university to get admission documents.
- Reach an application receiving agency for a recommendation.
- Fill in and submit the Application Form for Chinese Government Scholarship online.
- The application receiving agency submits the recommended candidates list and their application documents.
- The CSC organises designated universities and experts to check eligibility and academic performance, then finalises the short-list.
- The CSC notifies the admission results to the application receiving agency, which then informs the applicant.
The receiving agency depends on the programme. Section I(I) says a bilateral programme application is filed with the dispatching department of the applicant's home country. Section I(II) says a University Program application is filed with the Chinese universities that run the programme. Section I(III) says designated scholarship programmes are filed with the international organisations concerned. The table in section I(III) gives the receiving agency for each of these:

What to prepare before you start
Section VI of the guide sets two rules that affect the form.
- The passport: you must send a clear scan of the information page of your ordinary passport. If the passport is valid for less than 12 months from the expected start date of study, renew it and use the new one. If you could not apply for a passport for objective reasons, you may submit other documents that show your full English name, gender, nationality and date of birth, with the approval of the application receiving agency.
- The pre-admission document: every applicant must provide one, issued by the university applied to. Section VI gives two examples: a pre-admission notice from the university's international students admission department, or an invitation letter from the school or a professor.
Section VI also lists the diploma, academic records, language certificates and a study plan. The full list is in section VI.
How the online system works on screen
The MOFCOM Scholarship-CSC Program publishes its own instructions for its online system (Attachment 1). The steps below come from that programme's instructions. They do not describe the Chinese Government Scholarship in general.
- Go to campuschina.org and click "Scholarship Application for Students".
- Click "CREATE AN ACCOUNT" and register.
- Log in with that account.
- Click "Edit Personal Details".
- Fill in all the personal information.
- Verify the information and save it.
- Click "Finish" to return to the previous page.
- Select the Program Category "Type A".
- Enter the Agency Number 00010.
- Fill in the application information.
- Upload the supporting documents.
- Click "Submit".
- Click "Print the Application Form" and download the form.
- Submit the application as the dispatching authority requires.
The instructions say that the step 13 print and download happens after you submit. They also say that personal details and application information cannot be changed after submission. You may withdraw the application before the processing authority processes it, then edit and submit it again. Once the application is processed, it cannot be withdrawn.
The instructions say that an application with an incorrect agency number will not be processed. They also say that the programme category and agency number are issued to qualified candidates by the scholarship receiving authorities. The login page of the Chinese Government Scholarship Information System shows two sign-in types, "Applicant/Student User" and "Other User", and a "CREATE AN ACCOUNT" link.
