How long do I have to get a residence permit in China?

You have 30 days from the date of your entry to apply for a residence permit, if your visa states that you must get one after entering China. The Exit and Entry Administration Law, Article 30, sets that period for any foreigner whose visa says a residence permit is required after entry, and Order No. 42, Article 36, repeats it for international students holding a study visa marked that way. You apply to the exit-entry administration of the public security organ in the place where you intend to live.
What the rules say
Three official texts carry the 30-day rule, each for its own scope.
The Exit and Entry Administration Law, Article 30, says that where a visa specifies that the holder needs to apply for a residence permit after entry, the holder shall, within 30 days from the date of entry, apply to the exit/entry administrations of public security organs under local people's governments at or above the county level in the proposed place of residence. The Law speaks of foreigners generally; it does not name students in this article.
Order No. 42 (the Measures for the Enrolment and Education of International Students by Schools), Article 36, applies the same period to international students: where the study-type visa held by an international student states that a residence permit is required after entry, the student shall, within 30 days from the date of entry, apply to the exit/entry administration of the public security organ in the intended place of residence for a study-type foreigner's residence permit. Order No. 42 is published in Chinese; the English wording here is this site's translation.
The Regulations on Administration of the Entry and Exit of Foreigners, Article 9, covers the marking itself: where an applicant needs to obtain a residence permit after entry, the visa authority shall specify on the visa the time limit for applying for that permit after entry. In other words, the visa is what tells you the clock applies to you.
The Embassy of the People's Republic of China in the United Kingdom states on its own visa page that holders of D, J1, Q1, S1, X1 and Z visas with the "000" duration of stay shall, within 30 days from the date of entry, apply to the exit/entry administrations of public security organs at or above county level in the proposed place of residence for foreigners' residence permits. That is the wording of that embassy's page; other missions publish their own pages, and the texts read for this page do not state that every mission uses the same wording.
The National Immigration Administration's service guide for residence permits repeats the 30-day rule and where to apply, in Chinese, and adds one condition: an application for a residence permit valid for more than one year must be accompanied by a health certificate issued by the local health and quarantine department or a medical institution at or above county level, confirming that the applicant does not have a severe mental disorder, infectious tuberculosis, or another infectious disease that may cause major harm to public health. The English here is this site's translation of that Chinese text.
What "from the date of entry" means
The count starts on the day you entered China, not on the day your visa was issued and not on the day classes begin. The articles read for this page do not convert the 30 days into working days, do not state whether the day of arrival counts as day one, and do not state how the count interacts with weekends or public holidays. If you need the count applied to your own dates, ask the exit-entry office handling your case.
Where to apply, and what you bring
You apply to the exit-entry administration of the public security organ at or above county level in the place where you intend to reside; Order No. 42 describes the same office as the exit-entry administration of the public security organ in the intended place of residence. Under Article 30 of the Law, the application requires your passport or other international travel document, the information relevant to the specific application matter, and biometric identification information such as fingerprints. The same article says the exit-entry administration shall examine the application and make a decision within 15 days from the date of receipt.

What the official pages do not say
The articles read for this page do not say what happens to the 30-day count if you enter on a visa that carries no such marking. They do not state the validity of a study-type residence permit: Article 30 of the Law gives a minimum of 90 days and a maximum of five years for work-type permits and a minimum of 180 days and a maximum of five years for non-work-type permits, and the articles read for this page do not state which of those two a study-type permit falls under. They also do not describe any extension, replacement or reissue procedure for that permit.

Is the 30 days counted from the date on my visa?
No. Both Article 30 of the Law and Article 36 of Order No. 42 run the period "from the date of entry". The Regulations, Article 9, only require the visa authority to state on the visa the time limit for applying after entry.
My X1 visa shows "000" as the duration of stay. Does the 30 days still apply?
The Embassy of the People's Republic of China in the United Kingdom states on its visa page that holders of X1 visas with the "000" duration of stay shall apply for a residence permit within 30 days from the date of entry. That is that embassy's statement; for your own case, follow the instructions of the mission that issued your visa and of the exit-entry office.
Do I need a medical certificate?
The National Immigration Administration's service guide requires one only for a residence permit valid for more than one year: a health certificate from the local health and quarantine department or a medical institution at or above county level, confirming no severe mental disorder, no infectious tuberculosis and no other infectious disease that may cause major harm to public health. The articles read for this page do not state a document requirement for permits of one year or less.
How long does the office take to decide?
Article 30 of the Law says the exit-entry administration shall conduct examination and make a decision within 15 days from the date of receipt of the application.
